Description Our client, a respected nonprofit organization dedicated to providing essential healthcare services, is seeking a Patient Care Coordinator to join their compassionate and driven team. As the Patient Care Coordinator, you'll act as the primary link between patients, families, and healthcare providers, ensuring smooth communication and efficient care delivery. Your ability to approach challenges with empathy, precision, and professionalism will be key to success in this role. Key Responsibilities + Serve as the first point of contact for patients, assisting them with scheduling appointments, managing paperwork, and answering inquiries. + Facilitate communication between patients, caregivers, and healthcare providers to ensure comprehensive and seamless care plans. + Maintain accurate patient records, ensuring all information is up-to-date in compliance with HIPAA regulations + Connect patients with community resources, specialists, or other organizations as needed. + Monitor patient feedback and address concerns to improve service quality and enhance patient satisfaction. Requirements + Prior experience in healthcare, nonprofit, or customer service roles (typically 2+ years). + Exceptional communication and interpersonal skills, with a strong commitment to customer service and confidentiality. + Ability to multitask in a fast-paced environment while maintaining attention to detail and accuracy. + Proficiency in Microsoft Office Suite and familiarity with EHR (Electronic Health Record) systems is preferred. + Understanding of HIPAA regulations and patient privacy laws. Preferred Skills + Knowledge of healthcare processes or nonprofit services. + A degree or certification in healthcare administration, social work, or a related field is a plus. + Bilingual skills in English and another language are highly desirable but not required.
